Necessary PaperworkPlease download, print, and read the forms on this page in order to complete them. When we meet for our "Perfect Fit" consultation, you will already have your forms completed so that the pet sitter can spend time getting to know you and your pet(s). All forms are necessary except for the Key Retention and Medication Waiver forms. We only need the Key Retention form if we are keeping your key in our lockbox. We keep some clients' keys so that they can call us for services without having to get their key to us prior to pet care. We only need the Medication Waiver if your pet is on any type of medication. **************************************************************************************** We are experimenting with online forms. We do not have an online form for the Vet Authorization yet, so you'll have to download and print off a copy of that (see files to download on the right.)
